自1994年起,阮綜合醫療社團法人醫院設置專科助理一職,隸屬於醫療各科系,直至2002年改為隷屬護理部並更名為專科護理師,2018年10月22日正式成立專師部。
設有專科護理師培育計畫暨執業規範專責委員會,委員會之主任委員由醫療副院長主持,委員成員含蓋醫療部、護理部、人資室、專師部,其中專科護理師佔委員會人數比例>50%,管理專師及訓練專師執業與勞動權益維護。


壹、[專科理師角色與功能]

專科護理師係由機構聘請之執業進階護理師,其主要任務在於與醫師共同提供連續性及整合性的護理與醫療照護。

一、專科護理師應具備六大核心能力則分別為照護、教學、諮詢、協調、領導和研發能力,其內涵與指標如下

1.照護能力:運用進階整合性評估技巧,擬定及參與臨床照護治療計畫,協助處理複雜、特殊或困難病人整合性與個別化的照護問題。
2.教學能力:提供病人及主要照顧者健康教育指導,提升對疾病照護及健康促進的認知及自我效能。
      協助醫療團隊專業照護之教育訓練,促進團隊人員專業學術交流。
3.諮詢能力:提供病人、主要照顧者及醫療團隊專業照護諮詢,發展並持續治療性合作關係,共同解決醫療照護問題。
4.協調能力:以個案為中心,統合運用醫療資源,進行跨領域團隊溝通與合作,提供一致性優質照護計畫及安全就醫環境。
5.領導能力:終身學習並運用品質促進及危機處理機制,創造全人、全家、全隊及全社區之健康醫療照護系統。
6.研發能力:運用科學方法分析,發現臨床問題,以實證為基礎進行專業研究,建構及發展創新照護技術、知識及標準。

二、專科護理師專責培訓醫院提供依衛福部規定所制定的訓練計劃,培育內、外科訓練中專師通過甄試。


貳、[團隊成員]

本團隊包含內科、外科、婦產科、兒科、骨科、麻醉科專科護理師等共同與醫師照護。


叁、[核心服務項目]

業務包含:
一、醫療輔助行為;
二、健康問題之護理評估;
三、護理指導及諮詢;
四、預防保健之護理措施。
並明定除得執行第一項業務外,並得於醫師監督下執行醫療業務。
在醫院成立專科護理師執業委員會制定Clinical Nursing Guideline 或Clinical Nursing Pathway。以維護病患安全與品質。

1.提供臨床健康照護Providing Clinical health care
1. 依據專科護理師相關之法規及政策執行健康照護業務。 Provide healthcare services following the guidelines and regulations of the Nurse Practitioner profession.
2. 針對個案的健康問題進行病史訪談、健康評估、區辨診斷、疾病治療計畫、監測治療結果、並管理急性或慢性個案的健康問題。 With a focus on client’s health problems, NPs should implement history taking, physical assessment, differential diagnosis, disease treatment plan, and monitor care outcomes. Additionally, NPs should manage acute or chronic health problems of clients.
3. 依據實證資料提供個案優質與安 全的健康照護。 Provide excellent and safe care based on evidence-based practice.
4. 確實記錄個案的個別需求及健康照護過程。 Creating documentation of a client’s needs and health care process.
5. 提供個案全方位健康照護服務, 包括健康促進、疾病預防、減輕痛苦、指導和諮詢、疾病管理,維護和恢復健康。 Provide patient-focused care services, including health promotion, disease prevention, reducing suffering, teaching and consultation, managing diseases, maintaining and recovering health.
6. 運用證據為導向的臨床推理方式,向個案解釋檢驗、診斷及病情,以減輕個案的疑慮。 Utilize evidence in clinical reasoning when explaining to patients their laboratory results, diagnosis and disease condition in order to reduce anxiety and confusion.
7. 個案問題超逾專科理師的執業範疇、領域、經驗或能力時,能及時諮詢授權之醫師或其他醫療人員。 When a client’s medical problems are beyond the scope of practice of the NP, the NP must seek advice from a licensed medical professional or doctor in a timely manner.
8. 運用實證數據評值個案治療計畫成效並給予回饋。 Use clinical evidence to measure the progress of a client’s treatment outcome and to give clients feedback about their condition.
9. 為個案規劃及執行以實證為基礎的健康促進和疾病預防措施。 Plan and implement evidence based health promotion and disease prevention measures for clients
10.對法律倫理道德困境有明確的識別力,並在必要時採取適當的行動(如與他人協商或尋求法律途徑)。 Be familiar with laws and ethics and have the ability to intervene when necessary (via negotiation or through a legal representative).
2.教育education
1.正確評估個案、社區和健康照護團隊之學習需求及優先順 序,進而提供健康照護教育。 Accurately evaluate each client,community and health care team’s learning needs and priorities to provide relevant healthcare education.
2. 利用適宜的學習理論,制定教育計劃並選擇適當的教學方法進行個案之教育和評值。 Utilize appropriate learning theories, create education plans, select proper teaching methods that is best for the client and implement education and evaluation.
3. 尋求教育機會,並積極參與專科護理師的專業發展。 Seek opportunities for learning and actively participate in professional development of NP.
4. 倡導並落實終身學習,以精進專業知能。 Advocate and carry out life-long learning to upgrade professional knowledge and skills
3.團隊合作Collaboration
1.與醫療團隊維持有效的溝通和良好的合作關係 Maintain effective communication and collaboration with multidisciplinary teams.
2.與醫療團隊協商,建立共識並合作達成共同照護之目標,改善健康照護效能及品質。 Collaborate with medical teams to achieve a common health care mission and goal in order to improve quality of care.
3.透過支持、指導、教育等措 施,與同事、學生及其他專業團隊,建立團隊合作機制,增進個案之健康照護成效。 Through giving support, guidance and education, create a sound working relationship with colleagues, students and other medical professionals in order to enhance the quality of care.
4.照護品質維護/研究Care quality/ Research
1. 整合現有最佳實證資料、臨床經驗、個案之偏好,以持續提高臨床照護品質。 Consolidate current best evidenced- based practice, clinical experience, and client’s preferences in order to continue to enhance quality of care in the clinical setting.
2. 收集專科護理師照護結果之相關資料,以評值照護成效。 Collect information regarding NP care outcomes for evaluating effectiveness of NP practice.
3. 以批判性思考分析、評估和改善個案健康照護品質。 Use critical thinking to evaluate and improve the quality of care for clients.
4. 運用科學的研究方法,進行專科護理師健康照護相關之研 究,包括資料收集、分析、結果與討論。 Use scientific methods to implement NP practice related researches through the process of data collection, analysis, results and discussion.
5. 分析導致不良事件發生的風險與各種可能的原因,並製定相關措施以降低失誤並避免未來類似事件再發生。 Analyze what contributes to unsafe situations and the different factors that can mitigate risk and reduce the chance of similar situations from occurring.
6. 運用同儕審查制度中提供的回饋,以促進照護成效並激勵自我成長。 Apply peer review processes to provide feedback with the aim of enhancing care quality and motivate self-growth
5.領導/專業成長Leadership/ Professional development
1. 專科護理師執行健康照護時能清楚地向個案說明其角色與職責。 When delivering health care to clients, NPs must be able to explain clearly what their role and responsibilities are to the client.
2. 向雇主、社會、民眾、立法 者、和政策制定者推廣專科護理師在健康照護服務的成效。 Promote the effectiveness of NP health care to employers, society, general people, lawmakers and policy makers.
3. 有能力承擔領導者的多元角色。 Be able to bear the responsibility of a leadership role.
4. 透過領導者的影響力促進個 案、社區、醫療團隊和政策制定者之合作,以改善健康照護品質。 Promote partnerships with clients, communities, medical professionals and lawmakers through leadership in order to enhance quality of care.
